Appointments
When you schedule your appointment, please provide your
insurance information to avoid registration delays at the time of your
appointment.
Payment is expected at the time of your office visit. If you are running late, please call us. If it is necessary to cancel your
appointment, please contact us 24 hours prior to your appointment. (409) 744-4030
What to Bring To Your Clinic Visit
• Current
prescription medication bottle(s), and any over the counter medications or
supplements
• Identification
• All
medical insurance information
• Co-payment/Payment
(Check, Cash, Credit Card)
Phone Calls
Please tell the person answering the phone the purpose of
your call. They may ask additional questions in order to determine the severity
of your problem. If someone is not
immediately available to answer your questions concerning your healthcare
please leave all phone numbers where you can be reached. We will make every
effort to return your call in a timely manner.
For your convenience, UTMB Call Center Nurses are available
to assist you with your health care needs after hours, weekends, and holidays
by calling the clinic number, (409)744-4030.
Please provide them with as much information as possible so they may properly
assess your medical need and/or contact the physician on-call.
Lab Results
We want you to be aware of all results of labs/tests done
through our office. As our patient, you
can expect to receive a letter or be contacted by your provider concerning your
test results. If there is an urgent lab value, you will be contacted by
phone.
Prescription Refills
We encourage you to inform your provider during your clinic
visit of any medications you may need refilled.
If the need arises for a refill between office visits, please call your
pharmacy first. In the event you need to
contact our office for a refill, your request will be handled by our
staff. To ensure your request is handled
efficiently, please provide the following information: Name, date of birth, and
the UH number of the Patient; name and dosage of the medication; quantity
requested; last time medication was filled; name and phone number of pharmacy;
a call back number, including an alternate number. Please allow 48 hours for a response on your
refill request.
Referrals
As part of your treatment plan, your physician may refer you
to a specialist. For your convenience,
our staff will handle the processing of your referral, and any authorizations
required by insurance carriers. Please contact
our office if an appointment has not been scheduled within one week. Our staff
will gladly process any/all requests for referral updates for our
patients. Please allow five days to
complete this process.
